The "golden moment" is a well-understood concept for most rescuers. It describes the time from injury of an injured person to delivery of the injured person to a medical facility (in the most traumatic situation), that is, the operating room.

This concept originated in the battlefield of World War I and was later recorded by R. Adams Cowley , a surgeon from Maryland, USA (among many people).

Recent research has challenged the golden moment, thinking that prime time cannot be applied to all major injuries. Since one hour is considered too prescriptive, in fact it is often heard to call it the "golden moment."

Another factor is not only to send the injured to medical institutions, but more importantly to send the injured to the most suitable medical institution (eg, a major emergency center). This process may prolong the time for transferring the injured, and this It is usually the best time to treat the injured.

Although the one-hour medical relativity debate is still ongoing, what has never been challenged is to treat the wounded in critical times and minimize the need for delays.

According to the golden moment theory, the rescuer has 20 minutes to stabilize the wounded, withdraw the injured from the evacuation and rescue the injured, which is still a relevant and important time frame for technical rescue workers.

One might argue that the concept of Golden Hour does not take into account the changes in vehicle design encountered by on-site rescuers, the construction and additional safety equipment (eg airbags / bags), but I think 20 minutes or Less than 20 minutes is still realistic.

Rescuers should (through training and preparation work) identify problems that cause delays in on-site rescue time and work hard to resolve these delays. The team work method promotes the use of synchronized activities (simultaneously completed tasks), which helps rescuers identify issues that delay the on-site rescue time.

There will always be accidents that are more complex, challenging and unavoidably delaying the on-site rescue time, but we should try to complete the rescue in 20 minutes (or less).
